Hormone Replacement Therapy
Hormone Replacement Therapy (HRT) remains a cornerstone in the management of moderate to severe vasomotor symptoms and other estrogen-deficiency-related manifestations associated with menopause.
As ovarian function declines, the resulting hypoestrogenic state can lead to hot flashes, night sweats, vaginal atrophy, mood disturbances, and an increased risk of osteoporosis and cardiovascular disease in some individuals.
HRT involves the administration of estrogen alone (in women without a uterus) or in combination with a progestogen (in women with an intact uterus) to mitigate the risks of endometrial hyperplasia. Transdermal, oral, and vaginal routes are available, and the choice of formulation should be tailored to the patient’s symptom profile, risk factors, and personal preferences.
The decision to initiate HRT should be individualized, taking into account age, time since menopause onset, cardiovascular risk, breast cancer history, and other comorbidities. Current evidence supports the safety of HRT in healthy women who initiate therapy before the age of 60 or within 10 years of menopause onset, particularly for symptom relief and fracture prevention. The lowest effective dose for the shortest duration necessary remains the guiding principle, although some women may benefit from longer-term therapy under close clinical supervision.
While past concerns regarding HRT and cardiovascular or breast cancer risk, particularly those raised by the Women's Health Initiative (WHI), prompted widespread caution, more nuanced interpretations of the data now support a more individualized, evidence-based approach.
When appropriately prescribed and monitored, HRT can significantly improve the quality of life and reduce long-term health risks in menopausal women.
